We are seeking a Licensed Social Worker (LSW) to join our dedicated team at MVH Caribou, caring for Maine’s Veteran heroes, their spouses, and Gold Star Parents.

The Licensed Social Worker is responsible for the psychosocial wellness of the residents, and actively participates in resident assessment and discharge planning, allowing the resident to optimize available resources and transition successfully throughout their care experience.

Candidates must have a current and valid Social Work License from the State of Maine (conditional licenses will be considered). The ideal candidate will have the ability to effectively communicate, verbally and in writing, with a wide range of people, including the ability to communicate pertinent information about a resident's condition in order to collaborate with other disciplines both within and outside MVH.

Requirements:

  • Minimum of a Bachelor's Degree and a current State of Maine Social Work license (conditional licenses will be considered).
  • Long-term care experience preferred.
  • Knowledge of VA, Medicare & MaineCare guidelines preferred.
  • Experience with case management, care planning and discharge planning process is preferred.
  • Requires proficiency in spoken and written English.
  • Basic computer skills
