Job Purpose/Summary

The PRO OCM Generalist is considered an integral member of the PRO-OPTIONS Care Program (OCP), one of the social care services provided through the Social Care and Volunteer Initiatives ME of WFS. The position is expected to work in seamless collaboration with and across multiple OPTIONS professional caregiving teams and/or social care specialties. The PRO OCM Generalist performs a key role in facilitating informed and data-driven decision-making and building/maintaining a milieu of comfort, trust, and collaboration within the unit, as well as with affiliated partners, services, or programs, including with the direct beneficiaries participating in OCP (e.g., professionals operate in close partnership with seniors and their informal caregivers in planning and supporting their long-term caregiving needs and aging-in-place goals). The incumbent is expected to possess the abilities to apply foundational knowledge, skills, and professional concepts to complete and deliver substantive person-centered care and assignments, as well as to work on acquiring knowledge/skills to perform professional caregiving care/services based on the “whole person” and biopsychosocial practices, approaches, and models.

All of the OCP positions require availability and the capacity to perform expected social care duties within home and community-based settings with some level of professional independence. The PRO OCM Generalist is expected to allocate their time primarily (i.e., 85% of the time) performing individual tasks that relate to the schedule delivery of direct OCM services.

This position is assigned to manage the whole-person care and social health of about 75 cases (care cases can range from an average minimum of 68 cases to an average maximum of 105 cases; these average ranges are subjected to modifications, including to be temporarily increased or decreased to accommodate program needs and trends).

Minimum Qualifications

Education: Bachelor’s degree in social work, occupational therapy, psychology, or a related field.

Experience:

• 1 year of experience with care, service coordination, resource counseling, or navigation preferred .

Experience with a minimum of 2 of the following:

• Working according to culturally and linguistically appropriate standards across diverse populations (e.g., older adults, consumers with disabilities, LGBTQ+, ethnicities, ESL groups, immigrants, etc.)
• Servicing/caring for the “whole person” and responding to the consumer’s care, health needs, preferences, and social contexts
• Navigating available care systems and resources (e.g., public and private benefits, social care assistance health systems, long-term care services, housing, nutrition, Medicare/Medicaid, private health insurances, etc.)
• Assisting with and supporting care/service transition planning and warm hand off of referrals
• Working within and across diverse community-based settings and contexts
• Social services experience with older adults and their families/partners, as well as service delivery practices to promote maximum self-determination, functioning, and quality of life
• Collaborative transdisciplinary teamwork (e.g., teams composed of different members and professions cooperating across disciplines or systems)

Licenses, Certifications & Clearances:

• Act 33/34 Clearances
• FBI Clearance (Dept. of Aging if lived outside of PA for 2 consecutive years)
• Valid State of PA Class 1 Drivers License
• Access to properly insured motor vehicle for fieldwork and a safe driving record

Essential Duties & Responsibilities

• Conducts, facilitates, or directs person-centered, functional, and/or Social Determinant of Health (SDH) needs assessments.
• Conducts, facilitates, or directs the development, implementation, and monitoring of care and service goals/plans.
• Assesses, monitors, tracks, and reports on the progress, experience, and well-being of seniors and their living contexts.
• Completes all substantive assignments within the scope of OCP responsibilities, including compliance with the extant performance matrix and recordkeeping policies and procedures.
• Keeps up-to-date and well-informed about the policies, practices, services, and health, functional, and social strengths/limitations involved in caregiving situations with older adults and their informal caregivers.
• Applies problem-solving strategies (e.g., analyzing issues of diverse scope and formulating strategies for solutions).
• Effectively promotes, facilitates, and empowers the engagement of seniors and their caregivers in all aspects of the care process.
• Maintains accountability and accuracy of care records (e.g., assessment, plans, service activities, eligibility, billing, reimbursement, etc.), documenting within the DHS SAMS (WellSky Aging and Disability) database system.
• Demonstrates commitment to the agency and the PRO-OCP mission, vision, and values by:
• Assuring excellence in quality and the performance of services
• Adhering, implementing, and promoting the “whole person” philosophy and approaches
• Maintaining acceptable yearly professional performance evaluation rating
• Attending training and BEST supervision schedules as required by applicable regulations, the agency, and program policies and procedures
• Assuring quality and fidelity in services and care delivery
• Actively participating in one’s own professional growth and development
• Maintaining consistent professional work ethics, including attendance, service, preventing instances of fraud, waste, and abuse, and assuring compliance with extant policies, procedures, and protocols
• Having a commitment to the safety and welfare of one’s work environment
• Actively participating to assure adherence to and with extant program’s performance matrix, as well as assuring a 90% or higher compliance performance in all audited matrix categories
• Assisting with and supporting care/service transition planning and warm handoff of referrals
