Job Purpose/Summary
The PRO OCM Generalist is considered an integral member of the PRO-OPTIONS Care Program (OCP), one of the social care services provided through the Social Care and Volunteer Initiatives ME of WFS. The position is expected to work in seamless collaboration with and across multiple OPTIONS professional caregiving teams and/or social care specialties. The PRO OCM Generalist performs a key role in facilitating informed and data-driven decision-making and building/maintaining a milieu of comfort, trust, and collaboration within the unit, as well as with affiliated partners, services, or programs, including with the direct beneficiaries participating in OCP (e.g., professionals operate in close partnership with seniors and their informal caregivers in planning and supporting their long-term caregiving needs and aging-in-place goals). The incumbent is expected to possess the abilities to apply foundational knowledge, skills, and professional concepts to complete and deliver substantive person-centered care and assignments, as well as to work on acquiring knowledge/skills to perform professional caregiving care/services based on the “whole person” and biopsychosocial practices, approaches, and models.
All of the OCP positions require availability and the capacity to perform expected social care duties within home and community-based settings with some level of professional independence. The PRO OCM Generalist is expected to allocate their time primarily (i.e., 85% of the time) performing individual tasks that relate to the schedule delivery of direct OCM services.
This position is assigned to manage the whole-person care and social health of about 75 cases (care cases can range from an average minimum of 68 cases to an average maximum of 105 cases; these average ranges are subjected to modifications, including to be temporarily increased or decreased to accommodate program needs and trends).
Minimum Qualifications
Education: Bachelor’s degree in social work, occupational therapy, psychology, or a related field.
